It’s the quiet air piece, I was told that resmed no longer makes this elbow but this is the one I like and it’s totally quiet. I feel compelled to include that I used my first one for about 7 months with proper cleaning and the filter in the elbow got very dirty still and had clogged up the little holes and started making a hissing sound when in use. I don’t know if it was from regular use or the dusty air quality in my home but I got a sinus infection I feel like from using it for to long without replacing, so all that said, try to replace this piece every six months if possible, my mask and hose still work fine. I added a better picture because the one listed is blurry.

Fit is perfect
Bought this for my husband CPAP. Unable to get them locally and it would have taken a week or two to get a replacement for the one he had through the company. This arrived and a couple days and fit perfectly. Would definitely purchase again.

For all intents and purposes, this is identical to the OEM part.
The picture on the Amazon page is of the older (quiet flow) elbow. Although is is indeed quieter, it gets clogged up much easier, and is harder to clean. These elbows have a tendency for the silicone anti-blowback flaps inside to get stiff and not close properly. When this happens, you machine starts thinking you have a mask leak, and can't readout how many events per hour you are experiencing. Replacing the neck fixes that.

Perfect Replacement
This is a Resmed OEM replacement elbow that is a direct fit for the AIRFIT 20 full face mask. Seller shipped fast actually arrived before estimated day. Made a big difference in how my CPAP operates. It’s a lot quieter now and maintains the correct pressure throughout the night.

Hard to breathe
This seems silly but I found it harder to breathe with this elbow. It was also pretty leaky -- no matter what I did, I could feel the air flow on the outside. Maybe I just got a bad one, but as soon as I switched back to my older one, I immediately felt better.
