What are the ingredients?

The bottle says active ingredients are "Chlorhexidine gluconate solution 4.0% w/v.....Anticeptic." "Inactive ingredients: fragrance, gluconoactone, Isopropyl alcohol 4.0% w/v, lauramine oxide, poloxamer 237, purified water and Red 40." It is meant for cleansing before surgery. Regular use can be detrimental to a septic system. I use it occasionally to keep a bacterial skin fungus away.

What type of safety seal does the 4 oz bottle have?

When I brought mines the whole bottle came wrapped/sealed in plastic

I have the 4 oz size which I haven't opened yet. And YES, it is SEALED! I will be using it for skin cleansing prior to surgery because my surgeon and the hospital both require it.

Can you use this on the head as shampoo

I would not use as a shampoo. Does not really bubble . It is very thin like water. It will really sting if it gets in your eyes. It is used for prevention of staph germs on skin. My husband was required to use it before out patient surgery.
