The head holder doesn't rotate, do i have to remove the screw first?

When you install it , you can adjust the angle of the showerhead holder by slackening the fixing screwing. It cannot be left loose as it would eventually fall out . So really it’s a one position showerhead holder that can be positioned at an angle of your choice .

Will it clamp onto a plastic shower pole?

It should do. The clamping mechanism is pretty simple but effective and works fine on my chrome finish pole.

My shower head is super heavy , so I might not want it to rotate. Is there a way to fix the anger permanently?

My advice would be to buy a heavy duty model if your shower head is heavy. Mine is a plastic shower head and it lost tension within 6 months.
