It doesn’t go fully flat but you could push it back mine sticks out about 10-15 cm from the wall

Can't see why not. As long as shelf can take the weight of the monitor

whats diameter of the grommet hole need to be?

42mm sounds about right. I expected this to need ~10mm hole to put a bolt through. However, the clamp detaches and you have to feed part of it through your desk and then reattach it. I made a rectangular hole roughly 40mmX8mm. I wish I'd known this before purchasing but it worked fine in the end. It's a different design than most of the others I'd looked at on Amazon
