1 of my cats naturally has a low appetite, do you think the play food dispenser will lower it anymore than it is already since she has to work for it?

Add some appetite stimulant in too. The mix of food and a treat every so often may be more enticing. Plus cats don't like food sitting in their bowl. This helps with that issue

What is stopping the dispenser holes from facing down and dumping a bunch of food out?

We have a 30 lb dog that is food addicted. Her food is small and have never had the problem you seem concerned about. We usually fill thru the holes because it is very inconvenient to remove cylinder, fill and put back on.
