Are both sides of the shoulder pad smooth, or is one side of a material that will not slide off my shoulder?

The strap does have a pad with a non-skid side. I have had surgery on both shoulders & this strap has worked for me. The strap has been perfect.

Will this work as a replacement strap for a guitar gig bag?

As long as your bag has two rings that this strap can clip on to … yes

Are the metal hooks of good quality?

The metal hooks are very strong and of good quality. I use them for my dogs travel backpack, she weighs 15 lbs and these straps hold very well.
