Can you use an aux cable from the bluetooth adapter to the aux port of your car -- rather than streaming the music over fm?

It does work but is finicky. Since the aux port does input and output, I found the AUX cable needs to be plugged in before you turn it on or else it will throw the AUX port into input mode, expecting input. I had a bit of a fit getting it to work until I realized this.

Does this drain the car battery a lot or not really? Does it have an auto off or a manual off feature when not being used?

Just purchased this item in less than a week and my car battery died. There’s a meter that tells you your battery voltage but not sure how accurate that is. I’m at the auto shop right now and if it comes up that that battery was drained, I’ll be returning this item immediately!

Does the radio have to be on the same station as the device? Can I listen to local radio stations and still receive calls through Bluetooth?

no - you need to match the bluetooth freq on the device to an unused freq on your radio. One way around it is to tune the bluetooth device to your radio then listen to your local radio through your browser on your phone - counter-intuitive. I know, but its a way to get what you are asking for!.
