It’s the diameter of the end of the walking stick.
Remove the existing rubber foot and use a ruler to measure the diameter.
The new one will be a tight fit, apply a thin smear of washing up liquid to ease the new foot on.

What’s the difference between the crutch ferrule and the walking stick ferrule? Do the black bits on the bellows do anything?

Hello, the difference is the black rings on the bellows (which are in any case removable). When these are installed they make the bellows stiffer - this makes them work better on crutches which generally have to support heavier weights.
