can you assemble this without any or all of the shelves?

Yes, the shelves are removable and you can assemble them or not as you need.

The device shelf at the top is made from what material? I assume the other two shelves below TV are made from metal.

This adjustable TV trolley is made from powder-coated steel, so does the device shelf at the top.

What is maximum adjustable height possible for middle of the tv?

The middle metal hanging board is fixed to the mainframe by screwing down. Two holders allow the screen to tilt for different viewing angles at ±13°. And the hanging board can be moved up and down from 43.3inch to 63inch as you need. Hope this will help you.

I am having trouble mounting my 32 inch visio

If you follow the directions that are enclosed and have 2 people doing it and use the 2 black wing nuts to adjust the TV you shouldn't have any problems
