Do you work it in with a brush after you pour it onto cracks?

I did not, just fill crack and let dry.

I have a couple of 1" round x 1/2" deep holes in my asphalt driveway. Would this be a good product to fill them?

Yes. This will work for that. You may need to apply in layers. I've tried this on cracks in my asphalt drive and it's not good for those as this stuff is not elastic as one would lead to believe, the cracks just come back. Bad for cracks, holes should be ok.

Does the gallon size come with an applicator spout?

Yes, it is part of the cap so once you cut it to the size opening you want to poor/squeeze the crack filler you have to use it because the cover can never again seal the container. I have covered the opened cap with foil for a day and it has been fine but I try to finish using the gallon within about 48 hrs..
