Is it made with real walnut wood or just have a walnut design on pine

Box is made of MDF, Google it, I call it press board, dont see any walnut or even pine, would never buy again!

Could you fit tuxedo buttons, or a flag pin, where the cufflinks go?

Those both should fit just fine, very versatile

A selection of my watches are 48-54 mm will this case accommodate this size of watch

I think it would be really close, i won't recommend purchase for watches that large , that said you can always try and return item.

Can any of the watch holder compartments be taken out to hold something larger?

On this model no, it doesn’t look like it anyways. But they had a smaller one that had a different design on the top with like 4-6 watch spots and then an empty area next to it. This one has 12 watch spots and a draw for bands and cuff links. It does hold large watches. But I would say no the compartments do not come out.
