Has the problem with bottom bolt not staying in place been fixed ?

I easily solved the problem by placing a 2nd spacer on the bottom hex head. Just like the one on the top hex head bolt.

It's hard to tell. A single spring easily handles my wooden gate, but it's only 26 sq. ft., as opposed to your 60. Why not try using one spring, and then add a second if that proves inadequate?

No it will not matter which way the gate opens. It is set by the way you set the spring. One way will pull the gate closed the other will push it closed.
