To install hitch, you don't have to remove the upper 3rd bolt on each side of Xterra bumper but it does help to loosen those so you can swivel bumper slightly to lift hitch past the exhaust pipe, and to align holes in hitch and bumper for the new bolts. You install 6 new bolts with washers into threaded holes. I cleaned the rusty threads on the 2 exposed bolt holes a few days before attempting the installation. Be patient aligning holes - it's a 2 person job. Don't fully tighten any bolts until you have them ALL threaded in somewhat. A bright headlamp helps you visually align holes too.
