what is the smallest hole this will work on with the taper? Or the diameter of the taper (small dimension/large dimensions).

The small diameter on the taper is 3/16" and the large diameter is just shy of 5/16". The plunger itself will go through a 3/16" hole.

How do you adjust the tip to use on different size holes (to center it). Or is this just for one size??

The tapered tip allows for use on different size holes in the hinges. If the holes are too small for the tapered end to fit it will not work. If the holes are larger that the tapered end of this tool, it will not work either. The adjustment noted is only for spring travel or depth of indentation.

Is it easy to get an off-center marking? i have used similar tools and found that if the angle of the tool is off just a bit, the hole is not centered

Unlike other cheap punches, this Starrett will give you a dead center punch every time, not to mention that you can do it with only one hand? It's worth the price.
