Does this foot massager massage the heel of the foot?

Yes, I felt it massaged my heels better than the Miko Shiatsu Foot Massager, which I also tried, and is a pretty good massager, but it isn't designed for your foot to be totally immersed in it. This one fits over you ankle, like a short boot; the Miko fits more like a slipper.

Does this massage top of your feet and toes? I have arthritis, so top of my feet is where i need it most.

Does not massage the top of your feet. There's assure compression that is on top but not massage on the top of the feet anywhere. Sorry! But it still feels so good!! My foot is less than half the size to fit this thing and I still get inedible relief without getting much of the massaging evenly and I agree with you, add massage to the big toe where arthritis is

Does the air compression reach all the foot or just the toes?

I have petite feet, I only feel it sounds the top of my foot. It goes around my foot, but not really my toes, stops compression really at my ankle. Some around the heel but like I said I don't have an average size foot so no matter what, it's not going to work fully properly and meet my needs best because of that. But still, even with not getting the full amount of tlc, I'm hooked. Without air I'd suggest it!

It's relative - if you're using it in an otherwise quiet environment, you will hear it, but to my ear it's not particularly noisy or unpleasant - although, it *is* a motor noise and not Mozart, so..... If using it in a room where there's already some ambient noise (like my air conditioner, or the TV), I actually don't really notice it at all...
